What information should I have on hand?
Company Information
- Company name and business address (and mailing address if different)
- Chamber of Commerce registration number + SBI code (listed on your Chamber of Commerce extract)
- Payroll tax number
- Contact person's phone number and email address (no "info@" addresses—these are not accepted due to the GDPR)
- Number of employees, including part-time and on-call staff
Details of your Mandate insurance
- Name of insurance company: Mandaat Assuradeuren B.V.
- Policy number
- Whether your policy is a standard policy (with a deductible period) or a stop-loss policy
- Name and address of your insurance broker + account number
Select a package
- Check the box: SME FiT VOV — €159.00 per employee per year (excl. VAT). Or choose MKB FiT Compleet €143.00 per employee per year (excl. VAT)
- Desired start date: Always the first of the month, never retroactive—so the sooner you submit, the sooner you go live.
Do I need to cancel an existing occupational health and safety contract first?
If you already Occupational Health Service a contract with another Occupational Health Service , you must cancel it yourself, observing the notice period. Don’t have an occupational health contract yet? Then you can sign up right away.
Is this offer also available to self-employed individuals?
No, the MKBsubscriptions intended for employers with at least one employee. If you have questions about your specific situation, please call 088-277 8934.
What about the GDPR and medical data?
ZekerArbo as an independent consultant and is bound by a strict duty of confidentiality. Employees’ medical information is never disclosed to the employer.
By signing this registration form, you authorize ZekerArbo to:
- Carry out reintegration activities for employees on Sickness Absence after they are enrolled in the WIA;
- To share necessary administrative data for the purposes of claim assessment and reimbursement of intervention costs. This data is shared solely with insurers under whose policies the relevant (former) employee is covered through the (former) employer;
- Exchange reintegration data with Mandaat Sickness Absence (a division of Mandaat Assuradeuren B.V.), in accordance with the current protocol of the Dutch Association of Insurers and the Trade Association for Occupational Health and Safety Services.

All parties involved respect medical confidentiality and the legal regulations regarding privacy protection.
- If necessary, share WIA and amendment decisions with the insurance advisor’s pension department in case of questions regarding premium exemption calculations; premium exemption calculations;
- Share anonymized Sickness Absence WIA intake reports with an insurance advisor for the purpose of providing insurance advice.
- Access to policy information via the Insurers' Mandate.
In addition, you confirm that you have read and understood the statutory Mandatory Contract the associated terms and conditions of ZekerArbo.
You hereby declare that the employee association or the works council, if any, within your company has been informed of the above and that there are no objections to it.
You enter sick leave and (partial) return-to-work reports, as well as employee information, into Mandaat Assuradeuren’s Xpert Suite absence management system. This information is shared with ZekerArbo.
