Do you suffer from (threatening) health complaints or would you like to discuss something around work and health? Then the preventive consultation is for you. Together with a practice support worker or the company doctor you can discuss what is needed to prevent absenteeism and to stay healthy at work.
How does it work?
You can make an appointment for the preventive consultation yourself, without your employer having to know about it. This is your legal right! We treat your request confidentially, so your data will not be shared with your employer. Even the invoice is processed anonymously.
Do you want the company doctor to contact your employer to help find a solution? You can, but only if you give your explicit permission. And don't worry: no medical details are shared with your employer, it's purely about work-related solutions.
Are you currently reported sick?
If so, do not request a preventive consultation. This can cause confusion in the absence counseling process.

How do you request the consultation?
Very simple: fill in the form and click 'send'. You will then receive a confirmation via email with some additional questions, and we will contact you as soon as possible for an appointment.
Information for employers
As an employer, you can of course make your employee aware of the preventive consultation hour, but the employee decides whether to make an appointment. It is a voluntary choice. If the employee wishes, a feedback can be made to you as the employer. But again, this is done only at the employee's request and with the employee's approval.
